In other countries it’s either a case of :

-pay a (sometimes token) sum up front and get part of it reimbursed by the state later with the rest by your insurance company (like in France)

-have health insurance and hope for the best, for example that your particular illness is not excluded in the small print (like in the USA)

-make monthly payments to a health insurance scheme run in cooperation with the state which then pays for costs when ill in hospital or visits to the doctor – as in Germany, I believe?
